Who Qualifies
- Patients with no active health insurance coverage (including Medicaid/Medicare).
- Verification may be requested (one of the following:
- A signed Uninsured Patient Attestation Form (provided by Mobile Alice)
- A denial letter from an insurance carrier
- Proof of non-coverage
Discount Structure
- 25% discount off urgent care visits and select services for verified uninsured patients.
- Additional foundation support may cover part or all of a visit for patients who qualify for sliding scale assistance.
- Lab and medication costs may be billed separately at discounted partner rates.
Payment
- Discounted payments are due at the time of service.
- Payment plans may be arranged for higher-cost services.
